Advance directive resources for health care providersDorothy Garske Center's advance directives program was developed to help individuals and families prepare living wills and other health care directives. With the passage of the Federal Patient Self-Determination Act (PSDA), the Center began providing health care providers training and materials. The Center is no longer able to provide in-service programs. This page contains links to DGC resources for providers. Where practical, we provide reproducable forms that may be downloaded and used by your agency. In 1999/2000 the Arizona legislature added a mental health care power of attorney to the statute(Title 36 Chapters 36-3281 to 3287). This directive is intended to give individuals greater control over how mental health care treatment decisions are made.
